Consent for DT Vaccine

Dear Parents,

Please note that a medical camp to receive the DT (Diphtheria and Tetanus) vaccine will be conducted in the school on 24th November, 2025 for the students who have completed 10 years of age. A consent letter regarding the DT vaccine has been sent through your child.


Kindly fill in the consent form and submit it to the class teacher by Wednesday, 19th November.
